HealthSCOPE Benefits (With the Guardian DentalGuard Preferred Select PPO dental network)

All changes are effective January 1, 2018.  It is important that you communicate these changes to your providers in 2018.  If you are enrolled in the PH medical and/or dental plan, you will receive a new ID card.  All cards have been mailed to the mailing address we have on file.
You can also download the HealthSCOPE Benefits app, entitled MyHealthSCOPE, from the AppStore and download a copy of your ID card after January 1, 2018.  If you or your providers have any questions related to your new coverage, you can reach out to HealthSCOPE benefits at 800-804-1297.
